The Licensed Clinical Social Worker provides behavioral health counseling to patients and families.


  1. Assess, diagnosis, and provide treatment for patients using the DSM-5, for both substance abuse and mental health needs.
  2. Collaborate with an interdisciplinary team to develop appropriate supports and interventions, discharge planning, service coordination.
  3. Demonstrate proficient skill in evidence-based practices and theories to work with adults, children, and their families.
  4. Screens for high risk patients who need social service intervention.
  5. Assesses patients’ and families’ abilities and/or needs that may interfere with rehabilitation and medical progress.
  6. Assists patients and families to fully utilize community resources. Connects patients to HHC’s Community Health Worker and Licensed Social Worker.
  7. Documents social work intervention and charges in medical record within a timely manner.
  8. Counsels patient/family to achieve optimal level of psychosocial functioning.

  9. This position requires compliance with Health Center’s compliance standards, including its Standards of Conduct, Compliance Program, and policies and procedures. Such compliance will be an element considered as part of the Licensed Clinical Social Worker’s regular performance evaluation.
  10. Adhere to Hometown Health Center’s employee immunization policy.

While performing the job, duties may include bending, sitting, standing, walking, reaching. May also require lifting of patients and at times long hours. This position requires the ability to deal with stressful situations and crises; mentally demanding. Eye and hand coordination required.

Work presents potential exposure to infection, blood and other body fluids – must follow proper protective precautions. Exposure to drugs and narcotics, and other possible chemicals possible.


  1. Ability to make referrals to Community Agencies (when & how).
  2. Ability to provide ongoing communication with patients, families, physicians, customers.
  3. Can complete chart documentation.
  4. Knowledge of advance directive forms and legal lines of decision-making authority.
  5. Knowledge of high-risk screening criteria.
  6. Knowledge of Federal and State guidelines pertaining to all work areas.
  7. Knowledge of medical conditions and impact on patient care needs.


The following education requirements are considered essential:

  1. Master’s Degree in Social Work and licensed by the State of Maine.
