We are currently looking for a Part-Time Social Worker to work out of our Bangor office!
PRIMARY PURPOSE To provide clinical psychosocial assessments, supportive counseling, assistance with advance care planning, and/or patient centered advocacy to address social determinants of health and wellbeing for patients managing a medical event, condition or serious illness diagnosis. The goal is to augment success towards patient centered goals of care, empowerment of autonomy and self-determination, improvement in quality of life, and reduction in those social or emotional factors that impede either effective medical interventions, a patient’s rate of recovery, or success in navigating end-of-life care.
A successful Social Work candidate will provide the above services in our hospice, home health and/or palliative care programs and participate as a resource, educator, and member of the interdisciplinary team.
Education: Degree from an accredited school of social work or related degree with current LCSW, LMSW, LSW, LCPC or LMFT licensures, conditional licenses will also be considered.
Experience: One or more years of experience as a Clinical and/or Medical Social Worker, LCPC or LMFT in a health care setting preferred.
