Position Summary: The purpose of this position is to provide services and enhance the lives of the program participants we serve. This position coordinates programming in the Community Supports program that provides activity based services in the community for adults with intellectual disabilities. This is a Monday through Friday position with only occasional evening/weekend work (for activities that occur outside of typical working hours).
Program Duties:
- Conduct preplanning activities for Person Center Planning.
- Writes PCP Narratives to justify services and authorized units
- Conducts Comprehensive Functional Assessments.
- Attends PCP meetings for program participants
- Develops individualized service plans in conjunction with evidence practice and agency procedures and training.
- Enter PCP information into EIS.
- Makes purchases in preparation for events/activities
- Contributes necessary information for the Individual Service Plan in coordination with the Community Support Manager
- Acts as primary point of contact for external parties for all assigned participants. Checks in with guardians/SLP's/team members on a monthly basis to keep open lines of communication.
- Supports clients and their families in selecting quarterly and monthly activities to align with goals and personal financial budgets
- Create and maintains signed release of information documents.
- Oversees staffing schedules with participant programming needs.
- Coverage of shifts when scheduling conflicts occur.
- Facilitate ongoing communication with guardians, shared living providers, case managers, residential homes, Department of Health and Human Services and other involved parties.
- Provide daily monitoring of participant attendance
- Provide daily monitoring of staff documentation of services to each participant scheduled.
- Create standing orders for NET transportation
- Review and comment on GER's and Reportable Events, and ISP's.
- Participate in Reportable Event Review meetings and Adult Services meetings
- Comply and implement any agency or staff programmatic changes.
Supervisory Responsibilities:
- Administratively supervise all direct reports to ensure their compliance with their job responsibly and accountability.
- Verifies and approves time cards for direct reports. Approves time off requests per department policy.
- Provide all direct reports with monthly supervisions and annual evaluations and corrective actions when necessary. Conduct quarterly observations on each assigned staff member.
- Hold monthly staff meetings and participate in monthly team meetings.
- Oversight of staff development opportunities.
- Prepare and deliver 90 day and annual performance reviews.
General:
- Comply with all federal, state and local regulations as regards services for persons with developmental or intellectual disabilities or autism, including rights of recipients and mandatory reporting.
- Comply with Office of Aging and Disability Services and Department of Human Services regulations.

Qualifications:
- High School Diploma or equivalent
- Three to five years' experience in the field.
- Full Direct Support Professional and CRMA Certifications within 6 months of Hire
- Must have a valid driver's license, clean driving record and reliable transportation (up to date inspection, registration and insurance meeting Maine state requirements) to be available for use during work hours.
