An exceptional full-time (40 Hr./Wk.) opportunity is available for a Clinician to join our team of caring professionals in Lewiston providing quality mental health services as part of the ACT Team. Familiarity working with dual diagnosis population preferred. Shared Team responsibility for occasional rotational on-call phone coverage with stipend.


The ACT Case Manager is responsible for assisting persons with chronic mental health problems by providing rehabilitative and treatment services as well as environmental supports that promote the integration of the client into the community, sustain the client in their current living situation or another living situation of their choice, and enhance the quality of their life.


  • Provides community outreach and case management services while assisting clients in developing the knowledge and skills and acquiring the necessary supports to live as independently as possible in the least restrictive setting in the community. This includes: budgeting/money management, hygiene, housekeeping, nutrition, shopping, representative payee ships as needed and community integration activities.
  • Links to and provides support with other services (vocational, educational, social/recreational, transportation, etc.) and advocates for entitlements such as Maine care.
  • Provides interface between the ACT Team and other services providers throughout the state to specific clients.
  • Assists clients in obtaining non-psychiatric medical care.
  • Provides on-call coverage and works shift assignments which will include some after hours, weekends, and holidays.
  • Provides education with clients around symptom management, substance abuse, etc.
  • Helps clients identify strengths, set goals and develop a life plan; participates with team members in the development of a holistic assessment of the client, including physical, emotional, psychological, and spiritual.
  • Participates with client and other staff in the development and implementation of a plan that supports and advocates for the attainment of client's life goals.
  • Provides supportive counseling and psycho-educational services to individuals and others as needed, both on and off site.
  • Provides crisis intervention, assessment, referral, and follow-up in an environment safe both for the client and staff.
  • Collaborates with family members and significant others as appropriate.
  • Provides family education and consultation, if desired, by a person receiving community support services to help family members and/or other natural supports to develop support systems and help the person to manage their mental illness.
  • Meets established direct service expectations.
  • Utilizes the electronic health record (EHR) to ensure that documentation is complete, accurate, timely and meets regulatory and agency standards.
  • Participates in scheduled meetings, including case reviews, supervisory sessions, staff meetings and other inter-agency and intra-agency meetings as appropriate for program and clients.
  • Maintains and utilizes knowledge of community resources.
  • Provides public education and professional consultation services, when appropriate.
  • Uses personal vehicle as may be required for client transportation, outreach in the community, home visits, and/or agency related business.
  • Locates, uses, monitors, and/or develops community resources and services for clients.
  • Engages clients in developing rehabilitation plans and programming.

  • Minimum of MHRT/C Provisional certification required. Duties also require knowledge of parameters and needs of adults with severe and persistent mental illness equivalent to completion of a bachelor's degree in a human service and/or an equivalent combination of education and experience.
  • If providing substance abuse treatment, a minimum of CADC certification and experience with and knowledge of the addictive process is required, in addition to meeting the requirements for case management indicated above.
  • Must possess a valid State of Maine driver's license.
  • Absent a qualifying medical exemption and pursuant to Maine's Covid-19 Vaccine Mandate, being fully vaccinated as defined by Maine CDC, is a requirement for this position.
