CARE MANAGEMENT
Purpose of Position or Position Summary:
The Care Manager coordinates the care and service of selected patient population across the continuum of illness: promotes effective utilization and monitoring of health care resources; and assumes a leadership role with the interdisciplinary team to achieve optimal clinical and resource outcomes.
Scope of Service:
The Care Manager will provide quality case management services and discharge planning to assist patient and family in coping with illness and disability, facilitating timely return to the community. The Care Manager’s patients may range from newborn to the elderly and services are provided to all patients along the entire life continuum.
Education/License
2 years / Associate Degree in nursing
Experience
1-3 yrs.
Graduate of an accredited school of nursing.
Licensed to practice nursing in the State of Maine.
Three years’ experience in utilization review, discharge planning, and/or social work; knowledge of third part payer system and case management certification desirable.

Analytical, Computer, Customer Relations, Filing, Follow verbal instructions, Grammar / Spelling, Interpersonal, Medical Terminology, Organizational, Read/comprehend written instructions, Telephone, Typing, and Verbal
Strong clerical, organizational and time management skills needed. Must be able to communicate effectively with patients, families, interdisciplinary team members and payer. Possess the ability to remain objective and impartial.
