Minimum three years' experience working with consumers in a community-based setting; three years' experience as a case manager/coordinator is preferred


Job Description

Will be responsible for managing and coordinating comprehensive care for clients within the CCBHC framework. This role serves as one of the first contacts for individuals seeking services at the CCBHC. This role involves conducting screening, initial assessments, providing crisis intervention, and coordinating immediate care needs to ensure clients receive timely and appropriate services. This role works collaboratively with interdisciplinary care teams to ensure clients receive the necessary services and support to improve their health outcomes and quality of life.
