Medical Provider – Family Medicine or Pediatrics

Hometown Health Center (HHC) is growing! With a new, state-of-the-art facility currently under construction in Palmyra, we’re excited to expand our reach and continue providing exceptional care to Central Maine communities. Established in 2003, HHC is a Federally Qualified Health Center (FQHC) delivering more than 32,000 medical, dental and behavioral health visits annually to over 8,200 patients across Dexter, Newport and surrounding areas.

We’re currently seeking a Medical Provider (Family Medicine or Pediatrics) to join our dedicated team. This position offers flexible hours and the opportunity to provide comprehensive, patient-centered care in a collaborative, mission-driven environment.

About the Role

As a Medical Provider at HHC, you’ll deliver high-quality, compassionate primary care to individuals and families in our community. You'll work closely with a team of providers, nurses, behavioral health professionals and support staff to ensure patients receive holistic and coordinated care. This position may be based in Dexter, Newport/Palmyra (new facility coming soon!) or our School-Based Health Clinic in Newport, depending on your preference and organizational needs.

Key Responsibilities

  • Provide comprehensive primary and preventive care to patients of all ages
  • Diagnose and treat acute and chronic illnesses
  • Manage patient care plans and support long-term wellness goals
  • Collaborate with interdisciplinary care teams to ensure integrated care
  • Promote health education, prevention and early intervention strategies
  • Maintain accurate, timely documentation using electronic medical records (EMR)
  • Participate in quality improvement initiatives and team meetings

Minimum Qualifications

  • Valid medical license in the State of Maine
  • Board certification or eligibility in Family Medicine or Pediatrics
  • Strong interpersonal and communication skills
  • Experience in a clinical setting and comfort working with diverse populations
  • Commitment to providing high-quality, patient-centered care

Preferred Qualifications

  • Experience in community health or FQHC settings
  • Familiarity with electronic medical records and care coordination tools
  • Bilingual in English and Spanish
  • Interest in rural healthcare and reducing barriers to care
