About the Opportunity
The Clinical Quality Coordinator is responsible for developing, supporting and evaluating quality improvement (QI) and quality assurance (QA) activities across the organization. This role works closely with clinical teams, leadership and external partners to drive performance improvement in alignment with HRSA’s 19 program requirements, NCQA Patient-Centered Medical Home (PCMH) standards and other regulatory guidelines.
What You’ll Do
Quality Improvement & Clinical Outcomes
- Lead and support QI projects to improve clinical outcomes, patient experience and service efficiency
- Facilitate root cause analyses, PDSA cycles and team-based problem solving
- Review and monitor quality metrics such as UDS, ACO, PCMH and grant-related performance indicators
- Collaborate with providers and staff to interpret data and identify opportunities for improvement
- Assist clinical teams in developing and tracking progress toward quality goals
Data Analysis & Reporting
- Extract and analyze data from the Electronic Health Record (EHR) and other systems
- Prepare and distribute reports for internal leadership, HRSA, funders and collaborative partners
- Monitor trends and provide actionable insights to improve patient care and meet benchmarks
- Coordinate submissions for UDS, patient satisfaction and other required reports
Compliance & Accreditation Support
- Help maintain readiness for HRSA site visits, PCMH recognition and ACO participation
- Ensure QI activities align with applicable regulatory, grant and accreditation standards
- Maintain up-to-date quality-related policies, workflows and training documentation
- Work in collaboration with the Risk & Compliance Coordinator on shared priorities
Patient Safety & Risk
- Track and analyze patient incidents, complaints and near-misses
- Assist in identifying patterns and recommending corrective actions to improve safety
- Promote a culture of safety and support staff training on best practices
- Work in collaboration with the Risk & Compliance Coordinator on shared priorities
Team Collaboration & Training
- Support QI education efforts through training sessions, materials and coaching
- Engage staff in data-driven quality improvement and workflow redesign
- Participate in meetings with ACO, CCPM and other external quality networks
- Provides cross-coverage or input on overlapping initiatives in collaboration with the Compliance & Risk Coordinator
- Participates in the organization’s Quality & Risk Committees
What You’ll Need
Required
- 3–5 years of experience in healthcare quality improvement, clinical coordination, or compliance
- Knowledge of HRSA standards, PCMH guidelines and federal quality reporting (UDS, ACO, etc.)
- Proficiency in data tools (e.g., Excel, EHR reporting modules) and interpreting clinical data
- Strong communication, organizational and project management skills
- Ability to lead multidisciplinary initiatives and work independently
Preferred
- Bachelor’s degree in nursing, public health, healthcare administration, or related field
- Prior experience in an FQHC or community health environment
- Familiarity with quality improvement methodologies such as PDSA, Lean, or Six Sigma
