POSITION SUMMARY

Hometown Health Center is Hiring a Medication Refill/Prescription Assistance Program Specialist (Med Refills/PAP Assistant). This position is responsible for the administration of HHC’s Prescription Assistance Program which includes submitting all applications for review by the pharmaceutical companies, supporting our providers and patients by processing medication and related refills as needed or requested, and manages the 340B program.

The primary job duties for this position are:

  • Administers the PAP Program; updates and utilizes policies, procedures and protocols; provides ongoing updates to training manuals.
  • Works with staff and providers to identify and enroll clients who qualify for PAP; completes medication review for patient and work with provider for options through PAP; works with MA’s and Providers for signatures for PAP applications; contacts patients when medications are received,
  • Contacts qualified patients to provide education and guidance to initiate PAP process; provides guidance and/or assists patients with completing PAP applications and renewals.
  • Assists patients with no insurance to apply for free medications through the program.
  • Submits applications to Pharmaceutical Companies and document information appropriately in Data Net database and EHR.
  • Works with patients to obtain documentation necessary to initiate an “Appeal” when enrollment application is denied following through with guidance from PAP Coordinator/designee.
  • Assists patients who do not qualify for the PAP program to find other resources for reduced cost medications with a focus on utilization of 340B SunRX.
  • Manages 340B program keeping the Provider Panel current at all times.
  • Orders refills to ensure arrival in a timely fashion.
  • Documents and processes PAP medications according to policy
  • Labels all PAP medication per SFD policies, typed or handwritten, and ensure two person checks are completed prior to releasing medication from the PAP Department.
  • Completes monthly audits on Site Logs verifying medication been pick up.
  • Provides education and resource handouts to all staff and providers on the PAP program.

The ideal candidate would be self-directed, detail oriented and highly organized. In addition, the candidate would possess excellent communication and problem-solving skills and be able to work collaboratively with people of diverse cultures and lifestyles. Knowledge of generic and name brand medication and basic knowledge of medical terminology is essential.

EDUCATION and EXPERIENCE REQUIREMENTS

  • Minimum of two years of related experience or a combination of: Medical Assistant, Pharmacy Tech, or a Certified Nursing Assistant with certification to pass medication in a long-term care setting, or 1 year experience as an LPN.
  • Experience working with an electronic medical record.
  • Active license in Pharmacy Tech preferred.
