Summary:

The Per Diem Outcomes Specialist plays a key role in supporting quality care delivery and clinical excellence in home health services. This position provides expert consultation on OASIS documentation and clinical outcomes, ensuring accurate, comprehensive assessments that support appropriate care planning and reimbursement. The Outcomes Specialist collaborates across disciplines to promote continuity of care, reviewing start of care assessments and plans of care to align with best practices and agency standards. In partnership with clinical and quality teams, this role identifies trends in OASIS and coding accuracy, monitors for potential quality of care issues, and contributes to quality improvement initiatives. The Outcomes Specialist shares accountability for achieving the agency’s strategic goals related to quality and patient safety.

    Required Minimum Knowledge, Skills, and Abilities (KSAs)

    1. Education: Bachelor’s Degree in appropriate clinical designation/field with three years of clinical experience or education as a RN, PT, OT, or SLP with five years of clinical experience. One year of experience must be in home care.
    2. License/Certifications: Certificate for OASIS Specialist-Clinical (COS_C) certification or must obtain within twelve months of start date.

    4. Additional Skills/Requirements Required: N/A
    5. Additional Skills/Requirements Preferred: Demonstrated competency in OASIS and home health coding.
