The Progress Center has a Case Manager opening in our Oxford location. As a Case Manager at The Progress Center, you will be an integral part of connecting adults with intellectual and developmental disabilities with community resources that promote independence and foster community connections. Case Management Services aim to identify the medical, social, educational and other needs, identify the services necessary to meet those needs, and facilitate planning in accessing those services. Case management consists of intake/assessment, plan of care development, coordination/advocacy, monitoring, and evaluation.

Responsibilities:

  1. Complete assessment and/or coordination of assessment of the consumer's medical, social, educational, and other needs
  2. Develop and implement a plan of care under the direction of the consumer, his/her guardian and other team members
  3. Coordinate service providers and resources as described in the plan of care
  4. Link consumer to appropriate agencies, community resources and informal support systems
  5. Monitor the consumer's progress towards achievement of the objectives identified in the person centered plan, including evaluation of the plan itself on a regular basis
  6. Participate in staff meetings
  7. Perform other duties and responsibilities as pursuant to the policies and procedures manual and the supervisor.
  8. Update and maintain client information on electronic records.

Requirements:

  1. Bachelor's degree with specialization in psychology, behavioral health, social work, special education, counseling, rehabilitation, nursing or related field
  2. Proficiency with electronic software (Microsoft Office Suite) and electronic health records. Strong writing, typing and organizational skills are necessary. Strong interpersonal skills, and a professional presentation are required.
  3. Comply with all federal, state and local regulations as regards services for persons with developmental or intellectual disabilities or autism, including rights of recipients and mandatory reporting.
  4. Comply with Office of Aging and Disability Services and Department of Human Services regulations.
  5. Demonstrate respect and add value to the lives of those we support

Education:

  • Bachelor's (Required)
  • Experience in working with disabilities (Preferred)
  • Social Work License, LSW (Preferred not required)
  • Driver's License (Required)
