We’re currently hiring a Homeless Outreach Case Manager to support unhoused individuals across Kennebec and Somerset Counties. This independent, mobile role offers the opportunity to make a direct impact by connecting clients facing serious mental illness and/or substance use disorders with the services they need most.

What You’ll Do:

  • Provide street outreach and meet clients where they are—literally.
  • Build trusted relationships to empower individuals toward stability and wellness.
  • Use evidence-based practices to link clients to essential services: housing, healthcare, mental health, and more.
  • Collaborate with a supportive team while maintaining autonomy in the field.

What We’re Looking For:

  • Bachelor’s degree in Social Services, Mental Health, or a related field preferred but not required
  • MHRT/C or MHRT/C Provisional certification eligibility (info at MHRT/C Info).
  • Experience in billable case management preferred.
  • Valid Driver’s License and comfort with independent, mobile work.
  • Tech-savvy and adaptable.
