Job Summary:
To provide coordination in the oversight of the Minimum Data Set (MDS) and utilization management processes in accordance with current federal, state, and local standards, guidelines and regulations, our established policies, and procedures, to assure compliance with regulatory agencies and generally accepted clinical standards.
Essential Duties and Responsibilities
- Stays updated on federal and state regulations and professional standards, advising Nursing Unit Manager, ADON, or DON accordingly.
- Manages and monitors individual resident care plans, ensuring they address all identified needs and goals.
- Coordinates the review of nurse's notes to ensure adherence to care plans.
- Facilitates and implements procedures for annual RAI audits.
- Plans and schedules MDS meetings and ensure timely completion of resident MDS assessments.
- Participates in government agency surveys and assists with Nursing Supervision duties as needed.
- Adapts care practices to accommodate resident preferences and promotes person-directed care.
- Ensures accuracy and regulatory compliance of MDS/RAI documentation and conducts regular audits to maintain document accuracy.
- Assists with quality assurance of clinical documentation.
- Monitors documentation during care transitions and evaluates patient outcomes.
- Generates reports for Utilization Review Committee and addresses discrepancies.
- Identifies and addresses barriers to person-directed care.
- Assesses learning needs of residents and families.
- Provides information on available programs and demonstrates effective teaching skills.
- Evaluates effectiveness of education and facilitates shared decision-making.
- Orients and trains staff on regulatory compliance and required skills for RAI/MDS processes.
- Assists in developing and conducting in-service training programs.
- Oversees training of non-nursing staff involved in MDS and Care Plan processes.
- Coordinates nursing services with other departments and communicates assessments to the interdisciplinary team.
- Participates in self-evaluation and individual development planning.
- Stays informed of field advancements and completes required competencies.
- Attends safety educational sessions and advocates for safe practices.
- Articulates the importance of safety and prevention within the organizational model’s leadership qualities to support person-directed care.
- May float to other units or departments.
- Understands organizational safety goals and promotes their importance.
- Supports and promotes the mission and values of Covenant Health Ministry.
- Any other essential duties and responsibilities will be added between “Supports the mission” and “Other duties.”
- Maintains required licenses, certifications, and competencies, and completes annual compliance courses on time.
- Other duties as consistent with this role.
Job Requirements
Job Knowledge and Skills
- Knowledge and training in all aspects of LTC nursing, regulatory requirements, and reimbursement are preferred.
- Must be able to speak clearly, read, write, and comprehend the English language.

Education and Experience
- Associate or bachelor’s Degree or Diploma (Three-year program) in Nursing
- A valid State of Maine RN license
- MDS/RAC certification
- BCLS
An equivalent combination of education and experience which provides proficiency in the areas of responsibility listed above may be substituted for the above education and experience requirements.

Comp Range:
$33.43 - $50.15Rate of pay displayed reflects the beginning of the pay scale. At the time of an offer, determination of your offer will reflect your skills and experience as it relates to the position.
