DFD Russell Medical Centers (DFDRMC) operates four community health centers in central Maine. We are seeking a full-time Care Manager, in our Leeds location.
Our four health centers serve a multicultural, rural population of about 10,000 patients and have a family practice focus serving pediatrics to geriatrics. We have integrated behavioral health, care managers, patient assistance coordinators and an excellent clinical and clerical support staff.
The Care Manager will provide depression screening, health status, medication reconciliation, assist with transitions of care, perform annual wellness visits, weight management counseling, diabetes education, and advance care planning. Care will be coordinated with the patient's primary care provider, specialty practices and external support services.
A current RN license with the ability to practice in Maine is required. Previous Care Management experience is preferred. Knowledge of EMR is required, Centricity or Athena One is preferred. BLS is required, ACLS is preferred.
