Hometown Health Center (HHC) is seeking a skilled and mission-driven Clinical Quality Manager to support and lead quality improvement, patient safety, and compliance initiatives across our organization. This role is ideal for an RN who excels in performance improvement, data-driven decision making, and cross-team collaboration.

HHC is a Federally Qualified Health Center (FQHC) serving over 8,200 patients annually throughout Dexter, Newport, and our new Palmyra facility, opening soon. We provide compassionate, community-focused, high-quality care to individuals and families across rural Central Maine.

What You’ll Do

Quality Improvement & Outcomes

  • Lead and support QI projects to improve patient outcomes, safety, and experience
  • Facilitate PDSA cycles, root cause analyses, and team-based problem solving
  • Track and monitor UDS, ACO, PCMH, and other performance indicators
  • Partner with providers and clinical teams to identify improvement opportunities

Data & Reporting

  • Extract, analyze, and interpret clinical quality data from the EHR
  • Build and distribute reports for leadership, HRSA, ACO partners, and funders
  • Identify trends and propose action plans to support quality benchmarks
  • Coordinate required submissions including UDS and patient satisfaction reporting

Compliance & Accreditation

  • Support HRSA Operational Site Visit readiness
  • Maintain documentation for PCMH recognition standards
  • Ensure policies, workflows, and training materials remain compliant
  • Collaborate with the Risk & Compliance Coordinator on shared initiatives

Patient Safety & Risk

  • Review and track patient incidents, complaints, and near-misses
  • Assist with corrective action planning to strengthen safety practices
  • Promote a culture of safety through training and communication
  • Participate in Quality and Risk committees

Team Collaboration & Training

  • Provide training and coaching on QI methods and best practices
  • Support workflow redesign and change-management efforts
  • Engage in external quality networks such as ACO and CCPM partnerships

What You’ll Need

Required

  • Active RN license (unrestricted)
  • 3–5 years of experience in healthcare quality improvement, clinical coordination, or compliance
  • Knowledge of HRSA, PCMH, UDS, and ACO quality programs
  • Strong skills in data interpretation, EHR reporting, and Excel
  • Ability to lead projects and collaborate across departments
  • Excellent communication, organization, and problem-solving skills

Preferred

  • Experience in an FQHC or community health setting
  • Familiarity with PDSA, Lean, or Six Sigma methodologies
