Summary:

The Licensed Clinical Social Worker provides timely coordination of quality healthcare services to meet individual’s specific healthcare needs in a cost-effective manner; this involves adaptability to meet community members at health centers, in their home or other community sites. Provides and coordinates care with patients and involved persons for an appropriate plan that meets the medical, rehabilitative and financial needs of the individual. Works to provide community members support for social determinant of health barriers. Uses motivational interviewing techniques, advocacy skills and knowledge of mental health and community resources to overcome barriers, offering assistance of community organizations, other team members, and care coordination as appropriate. Applies social work methods and theory to the evaluation, diagnosis, treatment and prevention of psycho-social dysfunction, disability or impairment, including emotional and mental disorders. Provides emotional and mental health counseling to groups or individuals.

Requirements

Education, Training or Certifications Required:

1. Current Maine license in clinical social work-LCSW

2. Master’s degree in clinical social work for an accredited institution.

3. AHA Basic Life Support.

4. Valid Drivers License.

Experience Required:

1. Minimum two years of experience in the field of license.

2. Demonstrated knowledge of health, social service, and funding sources.

3. Experience in community and hospital social work and discharge planning preferred.

4. Experience counseling individuals and groups with behavioral, emotional and behavioral disorders.

5. Motivational interviewing experience preferred.

Special Requirements:

1. Within the scope of practice and license, must have the ability to work independently with limited supervision, capability to make independent decisions regarding counseling, education or referral of patients. Maintains confidentiality.

2. Reliable personal vehicle or mode of transportation.

1. Emotional maturity, resilience and stability.

2. Knowledge of best practice standards and ethics.

3. Ability to multitask, prioritize and organize.

4. Ability to form collaborative, proactive and patient-focused relationships to focus, facilitate and maximize client healthcare outcomes.

5. Demonstrated communication and leadership skills with healthcare team members to facilitate maximal client outcomes.

Physical Demands:

1. Able to sit, stand, walk for 8 hour shifts.

2. Able to carry 20 pounds.

3. Must communicate effectively, both verbally and written.

Environment:

1. Occasional potential for exposure to blood/body fluids.

2. High stress potential.

3. Frequent use of VDT and contact with third party payers.
