We are growing! Greater Portland Health is expanding its nursing team to support and increase our capacity to assess, educate, and empower our patients to live healthy lives. The Nurse Care Manager will provide care to patients of all ages with HIV, hepatitis, and latent TB. The NCM also serves in an expanded nursing role to collaborate with the Medical Team to provide a model of care that ensures the delivery of quality, efficiency, and cost-effective healthcare services.
The Nurse Care Manager will work with the team at 295 Park Ave in Portland, Maine. Scheduled hours will be Mondays or Tuesdays (8am-noon or 8am-430pm); Wednesdays 8am-430pm, and Fridays 8am-430pm.

Requirements:
- Minimum of two years of clinical experience with emphasis on HIV/AIDS patient care
- Registered Nurse with three to five years of recent (within the last 3years) acute care experience required.
- Licensed at an RN level by the State of Maine Board of Nursing.
- Experience in care management/care coordination to include care planning, resource management, and utilization management preferred.
