Penquis, a social services agency serving Penobscot, Piscataquis, and Knox counties for over 50 years, is currently seeking a full time (40hrs) Health Home Coordinator. As the Health Home Coordinator, you will provide care coordination/case management to children who have an approved mental health diagnosis. This position is remote, with travel and in-person meetings required in the Dover-Foxcroft and Bangor areas depending on the needs of the program.
Location: Bangor, Maine
The rate of pay for this position starts at $21.67-$24.00 per hour, as additional compensation may be given consideration with relevant experience.
