Reports to: Director, Quality, Safety, & Experience

Summary: The Quality Coordinator works under the auspices of Senior Leadership and the Director of Quality, Safety, & Experience to coordinate and support quality and safety initiatives. The quality coordinator works in the hospital, clinics, and Birch Bay Village to develop, maintain, and implement plans to achieve quality and safety goals. The coordinator identifies strategies and opportunities in collaboration with the healthcare team to design, implement, and evaluate clinical quality improvement initiatives and provide ongoing monitoring, coordination, and analysis of identified program initiatives. The coordinator will educate staff using research and evidence-based practice to move initiatives forward. The coordinator will work with the Leadership Team and Quality Director in participation with national and other initiatives as accepted by MDIH, including, but not limited to quality data extraction and timely reporting. The coordinator will support survey readiness initiatives to ensure compliance with accreditation and regulatory requirements. They willL leads and participates in special projects as directed, such as spearheading patient experience initiatives. The coordinator supports both Organizational Quality and Board Quality and Safety Committees’ designated improvement initiatives.

Requirements

Major Responsibility Areas for this position (All are Essential Functions):

#1- Coordinates Quality Improvement Initiatives to integrate and move processes forward including successful participation in external processes and reporting targets. Must be familiar with all parts of the medical record and chart review.
Measurement: Demonstrates high level knowledge of quality initiatives, attends pertinent educational sessions and provides documentation of quality initiatives coordinated.

#2 – Performs extensive medical record reviews of identified patient quality indicators and other reviews as assigned as part of current quality initiatives. Performs analysis of data related to Quality Initiatives.
Measurement: Demonstrated competency of data analysis and outcome monitoring as observed by the Quality Director.

#3 – Enters collected information into designated programs and software in order to meet both regulatory requirements and organizational needs. Disseminates information and develops planning and successful work tools. Reviews changes in reporting requirements, disseminates and provides updates to key stakeholders in a clear and concise manner to ensure the integrity of the reporting and extraction process.
Measurement: Data entry is complete, timely, and accurate and passes any external validations. Changes are reviewed and explained to stakeholders including the Organizational Quality Committee and Board Quality Committee Members.

#4 – Is proficient in all organizational Electronic Medical Records for the purpose of data analysis and collection and participation in Quality Projects including American College of Surgeons NSQIP, QHIP and Leapfrog Programs. Organizes effectively and is proactive in promotion of Quality Improvement. Reviews data findings and trends with key stakeholders and demonstrates outcomes and impact on overall quality to the organization.
Measurement: Quality Initiative data is accurate and meets deadlines.

#5 – Participates in Committee Assignments as assigned, collaborates with the Organizational Quality Committee and the Outpatient Quality Improvement team to integrate process improvements throughout the organization.
Measurement: 90% attendance in records of designated committees.

#6 – Develops, leads and promotes a unified quality program through building teams and meetings that are designed to provide an overview of the organization’s quality initiatives, deliverables and timelines to meet all quality objectives.

Measurement: Team meeting minutes and in house publicizing of quality initiatives.

#7 – Attends external meetings as requested by the Director of Quality, Safety, and Experience and brings the information back for presentation and implementation to MDIH.

Measurement: Observation by Director that information is complete and implementation imitated as delegated.

#8 - Participates and Coordinates Efficiency Risk Assessments within the Quality Scope as requested.

Measurement: Demonstrates the ability to be an effective facilitator.
