Join the Catholic Charities Maine Behavioral Health Network in Portland, Maine and help create positive change! We currently have one full-time opening for a Transitional Care Coordinator.

The Behavioral Health Network is dedicated to helping adults and families struggling with a mental health diagnosis and/or substance use disorders. We are an interdisciplinary team of psychiatrists, nurse care managers, clinicians, care coordinators, and peer support specialists who provide an integrated and mission-guided approach to client care for adults and families.

The BHN Transitional Care Coordinator shall be a trained, supervised professional who will provide Care Coordination Services within the Client Access Response and Engagement (C.A.R.E.S.) Team program. Other Key Responsibilities include:

  • Act as a collaborative bridge for individuals enrolled in all BHN programs.
  • Support a caseload of individuals needing assessment, stabilization, or help navigating life and service transitions.
  • Deliver direct support for:
    • Service requests
    • Pre-intake and orientation
    • Access to services
    • Outreach activities
    • Ongoing case support
  • Ensure compliance with state, agency, and program guidelines.
  • Provide support to individuals and families as part of the BHN and C.A.R.E.S. team.
  • Maintain professional training and supervision as required for the role.

Qualifications:

  • Must be certified by the Department as a Mental Health Rehabilitation Technician/Community (MHRT/C or with provisional A, B with work actively being completed toward C),
  • Must be motivated to obtain a CADC and CRMA (or ability to obtain CADC and CRMA within 6-months of hire) along with specific professional experience and training in working with adults, youth and families.
  • Shall have at least two (2) years of relevant experience working with adults and/or youth and families, specifically in a behavioral health case management capacity.
  • Must possess a working knowledge of pertinent local community resources.
  • Must have the requisite computer skills necessary to function successfully in an electronic health record environment

This position requires a Mental Health Rehabilitation Certificate (MHRT-C) or MHRT provisional certification. A MHRT-1 does not qualify. If you hold a bachelor's degree in a related or unrelated field you may be provisionally qualified.
