JOB SUMMARY:  

Restores patient’s function, alleviates pain, and prevents disabilities following disease, injury, or loss of a body part by planning and administering physical therapy treatment programs.

 

ESSENTIAL FUNCTIONS:

  1. Provide quality care by selecting appropriate evaluation and treatment methods taking into consideration the diagnosis, age group, and patients’ physical and emotional status and then interpreting those test results.
  2. Formulate an appropriate rehabilitation plan for each patient which includes long and short-term goals.
  3. Suggest equipment and assistive devices to help the patient attain maximum independence.
  4. Demonstrate essential skill competencies in the use of treatment modalities, equipment, and protocols.
  5. Perform re-evaluations according to department guidelines and adjust the treatment plan accordingly.
  6. Document patient care services by charting appropriately in the patients’ records.
  7. Provide concise and pertinent information regarding the patient to members of the health care team, including the physician.
  8. Completes discharge planning by consulting with the healthcare team and attending interdisciplinary team meetings.
  9. Assure continuation of the plan of care following discharge by designing home exercise programs; instructing patients, families, and caregivers as needed in home exercise program; recommending and/or providing assistive equipment; recommending outpatient or home health follow up programs.
  10. Use safety techniques at all times when working with patients.
  11. Protects patients and employees by adhering to Infection Control policies and procedures.
