The Nurse Care Manager - Maternal Child Health (MCH) oversees clinical care for perinatal patients and children. This position serves in an expanded nursing role that collaborates with the medical team to provide a model of care that ensures the delivery of quality, efficient, and cost-effective healthcare services.

How your time will be spent:

  • 60% Care Management
  • 40% Direct Patient Care

Required:

  • Registered Nurse with one to two years of Home Care, Ambulatory Care, or Acute Care experience preferred.
  • Licensed at an RN level by the State of Maine Board of Nursing.
  • Bachelor’s degree in nursing preferred.
  • BLS Certification.
  • Experience in care management/care coordination to include care planning, resource management, and utilization management desired.
