Cary Medical Center has an exciting opportunity for a Diabetes Educator to join our Specialty Clinic Diabetes team. This individual will provide quality education on managing diabetes following standards of the American Association of Diabetes Educators and will exhibit leadership skills and teamwork.

Job Summary: Delivers quality nursing care according to the standards within the ANA Standards of Clinical Practice and Maine Nurse Practice Act. Demonstrates competence in nursing assessment, diagnosis, planning, implementation and evaluation. Exhibits leadership skills and teamwork. Promotes patient advocacy and supports learning needs of staff and students. Collaborates with other disciplines to ensure effective and efficient patient care delivery and the achievement of desired patient outcomes. Supports the organization’s vision and mission. Utilizes knowledge of patient’s age and culture in provision of patient care. Maintains organizational and unit requirements. Promotes a collaborative and cooperative environment with emphasis on customer service. The Diabetes Education Program will have Diabetes Education Back-up Instructors to assist with the Diabetes program.

Essential Duties:

1. Completes assessment and recognizes actual and potential health problems.

2. Documented nursing assessment reflects review of patient’s psychosocial, spiritual, environmental, and health education status and data is reflected in the DSMES Data Record. (Diabetes Self- Management Education and Support)

3. Completes nursing assessment on Diabetes patients promptly and adequately at Education session.

4. Continuously assesses patient response to illness and intervention and ensures documentation of ongoing assessments is appropriate and complete.

5. Uses critical thinking skills to assess, anticipate and problem solve and identifies patient and family needs as expressed verbally and through behavior and initiates action to meet them.

Requirements

License Requirements:

1. A Registered Nurse licensed to practice in the State of Maine.

Availability Requirements:

1. Depending on department staffing needs, you may be asked to work on non-scheduled days for emergency needs and emerging situations in addition to scheduled time.

Education, Skills, Experience Requirements:

1. Meets a minimum of 15 hours per year of continuing education in diabetes or diabetes related topics.

2. Meets the unit’s annual competency training requirements.

3. Maintains current BLS certification.

4. Works toward CDCES within 2 years in position.
