The Licensed Practical Nurse (LPN) will provide nursing care; assist clinical staff while interacting with patients of all ages. The LPN will work under the under the supervision of the provider and Nurse Manager and will work as part of a Care Team to ensure effective and quality care is given to the KVHC patient, in accordance with nursing professional standards and KVHC protocols and procedures. RESPONSIBILITIES 1. Under the direction of the Nurse Manager and or Provider the LPN is responsible for providing medical care to patients in the Health Center setting. 2. Prepares for the days schedule with a team huddle with provider. 3. Triages patient as needed and takes patient vital signs and document. 4. Take histories and perform screening procedures as indicated including the Social Determinants as indicated for the Nurse. 5. Prepares exam room and treatment room with necessary instruments. 6. Assist Providers in preparing for minor surgeries and physicals. 7. Assists with tests such as EKG, as directed by the provider. 8. Administers parenteral medications including routine immunizations, therapeutic injections and inhalation medications via nebulizer under the direction of a provider. 9. Performs lab tests, e.g. HCT, urinalysis, pregnancy tests, also draw blood and prepare it for transport. 10. Provides Patient Education as needed. 11. Assists in maintaining compliance with quality assurance standards including CLIA and COLA. 12. Supports the pre-visit planning process and prepares for the days schedule with a daily team huddle with provider. 13. Participate in the Clinical Care Team as outlined in Team Approach to Care Protocol. 14. Work as part of the Clinical Care Team to ensure effective and quality care is given to the KVHC patient. 15. Maintain the laboratory and laboratory equipment in good order; maintain proper temperature in incubators, refrigerator and freezer, according to CLIA standards. 16. Prepares and maintains supplies and equipment for treatments, including sterilization. 17. Checks charts for lab and x-ray reports for review. 18. Performs additional duties or projects as assigned by the Nursing Director or Clinical Coordinator. 19. Adheres to all KVHC policies and protocols. 20. Complies with all safety rules and protocols. Reports all incidents, regardless of severity, immediately to their supervisor and completes an incident report and investigation form, which will be forwarded to Human Resources within 24 hours of the incident. 2 | P a g e Confidential Last updated: May 24, 2023 DRAFT 21. Conducts themselves as a good steward in the communities served by KVHC; represents the organization at outreach activities as needed. 22. Actively participates as committee member as assigned by the Chief Executive Officer. 23. Interacts harmoniously and effectively with others. It is always okay, and usually preferable, to pause, and take time to reflect before reacting to a situation. EDUCATION AND EXPERIENCE 1. High school diploma or equivalent, or a combination of education and experience. 2. Must be a graduate of an Accredited School of Nursing, and have a current license as a Licensed Practical Nurse by the state of Maine. 3. Must possess the knowledge of Maine State Board of Nursing regulations as applicable to licensure. 4. Must have a current CPR certification. 5. Previous Experience with Electronic Health Records preferred. 6. Minimum of 2 years’ experience in a health care setting. 7. Must possess excellent communication, problem solving and interpersonal skills. 8. Must possess the ability work in a high pressure health office setting. 9. Must possess excellent computer and keyboarding skills. 10. Must possess the ability to learn and adhere to the KVHC Safety and Compliance Policies and Protocols. 11. Valid State of Maine Driver’s license and ability to be insured by KVHC’s vehicle insurance carrier.
