The Schooner Health and Living Services RN performs tenant visits, assesses needs,
implements direct care, evaluates clinical outcomes, and planning for
future health and social needs. The RN coordinates
care with the tenant, their family, physicians, and other
providers for the Schooner Home Health Agency, with the unlicensed care
team delivering Activities of Daily Living (ADL) services to tenants in
apartments and assisted living studios. The RN may also provide first aid and other
non-skilled nursing services.
