Hometown Health Center is hiring a Community Health Worker (CHW). Hometown Health Center is a Federally Qualified Health Center in rural central Maine that provides comprehensive health care including medical, dental, and behavioral health services. The CHW position is responsible for helping patients and their families navigate and access community services and resources. The CHW supports the patient care team through an integrated approach to care management and community outreach, focusing on activities that promote, maintain, and improve the health of patients and their families. A CHW provides social support and informal counseling, advocates for individuals and community health needs. Join the team and make a difference in our Hometown Health community.

Experience and Skill Requirements: The following experience and skills are considered essential:

  • Ability to work with a diverse population, including adults, teenagers and children.
  • Demonstrated ability to work collaboratively with other team members.
  • Knowledge of community resources and state and federal assistance programs
  • Familiarity with the Social Determinants of Health
  • Ability to maintain confidentiality and comply with HIPAA requirements.
  • Excellent Communications Skills
  • Valid Driver’s License with an acceptable driving record.

Education Requirements:

  • High school graduate or equivalent required.
  • Successful completion of a Community Health Worker formal training program such as from a college or other education institution is preferred.
  • Associate's Degree in Human Services, or related field desirable or experience as an MT, Paramedic, or Certified Nursing Assistant or related medical background also applicable.
