Description

The Behavioral Health Home Organization Nurse Care Manager (BHHO NCM) carries out medical functions to all clients receiving services from the BHHO team. Medical functions include basic psychiatric health assessments, treatment, monitor medications, education, and coordination of health care provided to clients in the community. The BHHO NCM will also be responsible for assuming lead role in developing and directing rehabilitation services to a designated caseload of clients. These responsibilities include ensuring face-to-face contact is completed to ensure safety and quality treatment is being provided according to all regulations and laws pertaining to licensure in Maine with the State Board of Nursing, codes of ethics, MaineCare rules and regulations, HIPAA, and Agency policies and procedures and is within scope of practice and the client’s plan of care.


This position will be required to meet the requirements of a regular office schedule, working five days per week Monday-Friday, between the hours of 7am and 5pm.


Because the BHHO NCM will occasionally drive to a client’s home in order to provide services, proof of vehicle insurance at $50K/$100K and a valid Maine driver’s license are required.

Qualifications


Duties include and are not limited to:

  • Conducting quarterly audits of BHHO clients to comply with Section 92 MaineCare regulations
  • Monitoring medical and psychiatric medication side effects and coordinating with providers to support client in decreasing negative effects
  • Participating regularly in treatment planning meetings and in organization staff meetings
  • Visiting clients in their homes per their plan of care
  • Providing regular education to staff and clients according to medical and psychiatric needs
  • Providing direction in collaboration with psychiatric providers, psychiatric consultant, PCPs, and the PCP consultant
  • Developing, revising, maintaining, and supervising team psychopharmacology, medical treatments and medication policies and procedures
  • Adhering to all state regulations, policies and contract parameters
  • Attending all scheduled BHHO team meetings
  • Obtaining all necessary medical documents and signed releases for all medical diagnosis, lab orders and results, history and physicals, hospitalizations, hospital discharges when applicable
  • Providing primary care consultation, psychiatric consultation, and working with the BHHO, the HHP and the member to provide other Section 92 services as necessary and pursuant to the plan of care
  • Understanding and working within the Assistance Plus Human Resources, administrative and behavioral health policies and procedures
  • Working harmoniously with all agency staff and community members
  • Accepting shared responsibility and authority with other team members to successfully accomplish the goal of each client’s plan of care and other teams
  • Performing required amount of documentation in a timely fashion
  • Assuring quality in work performed in order to facilitate the delivery of quality services
  • Receiving notifications from HealthInfoNet (HIN)
  • Following up with client, case manager, and PCP after receiving HIN notification and completing necessary documentation
  • Being aware of and responsible for maintaining all requirements of the AMHI Consent Decree as it pertains to providing client services
  • Helping with BHH opens as needed
  • Performing other duties as assigned

Position Requirements

  • Proof of vehicle insurance at $50K/$100K and a valid Maine drivers license are required
  • Must be 18 years of age or older
  • Associate's or Bachelor's Degree in nursing is required
  • Minimum of three years clinical nursing experience is required
  • Must hold a current license to practice in the State of Maine as a Registered Nurse (RN), Advance Practice Nurse (APRN), or Clinical Nurse Specialist (CNS)
  • Intermediate level computer experience
