What You’ll Do:
- To provide clinical management by supervising and guiding staff in the provision of health services in the home and community.
- Responsible for oversight of personnel and all patient care services provided in the home healthcare program whether directly or under arrangement.
- Supervises: RNs, LPNs, PTs, OTs, STs, and Home Health Aides
- Responsible for assuring the development implementation, and updates of the individualized plans of care, which includes communication with all physicians involved in the plan of care including those orders related to medication
- Overall management of OASIS with strong focus on SOC, Recertifications & D/C OASIS
- Collaborates with team schedulers for scheduling
- Acts as liaison person with referral services and referring hospitals when indicated
- Serves as liaison for discharge planning when indicated
What You’ll Bring:
- Current RN licensure in the State of Maine.
- ANA certification in Home Health, Community Health nursing,
- A minimum of 3 - 4 years of progressively responsible experience in acute care nursing with one or more years of leadership experience in Home Healthcare preferred.
- Experience in a Home Health setting strongly encouraged.
- Gerontology, or Med/Surg nursing encouraged.
- Current CPR certification is required (must be Healthcare Provider). OASIS certification within 2 years of hire encouraged.
