Do you enjoy working with children and families? Do you enjoy partnering with a multi-disciplinary team to enhance outcomes? CHCS has an exciting opportunity to be part of a new School Based Tele-Behavioral Health Program. Join our agency to work side by side with a Licensed Clinician and School Staff to meet the behavioral health needs of youth in the Ellsworth Middle School setting. As a Community Health Worker (CHW), you will be primarily based in the school and will be instrumental in the success of this program. You will hold a key role in making a difference in the lives of students and their families through establishing partnerships; linking students and families to needed services; collecting data and conducting screenings. You will partner with the Clinician and School community to provide case coordination and you will have the opportunity to support Students in School and in their homes and community. Join our team in this new endeavor to increase supports for youth in need.
