Position Summary
The Triage Nurse provides assessment, triage, and recommends dispositions to patients during telephone calls using evidence-based protocols, Clear Triage, and independent assessments (within scope of practice). The Triage Nurse provides excellent and appropriate external and internal customer service in a timely manner resulting in improved patient care and satisfaction as well as more efficient use of available provider appointments, home recommendations, and decreased ER utilization.Job Description
Key Outcomes:
- Triages patient phone calls through a clinical call center queue.
- Triages telephone calls applying standard protocols for patient care and recommends dispositions, i.e. schedule an appointment, refer to emergency room, or offer home advice using appropriate critical thinking and Clear Triage tool (knowledge base/scope of practice to support the various types of patient population and services: Pediatrics, Family Practice, Internal Medicine, Surgical, Cardiac).
- Prioritizes and schedules acute primary care services to meet patient needs while maximizing available resources.
- Performs independent patient assessment to determine the level and urgency of primary care services, including pain assessment, sick visit assessments, and other health concerns applying standard protocols for disposition guidance.
- Provides education regarding diagnosis, procedures, medications, risk factor modifications, exercise prescription, and follow up activities/care to patients and/or family members by phone as directed by providers.
- Ensures accurate, timely documentation in the EMR (including scanning) per best practice guidelines or workflows.
Education/Experience:
- Associate degree in nursing; Bachelor’s degree in nursing preferred
- 3+ years’ relevant experience (triage nurse, home health nurse, clinical specialist, or other subspecialty)
- Telephone triage experience preferred
Required License(s) and/or Certification(s):
- Current, unrestricted Registered Nurse licensure in Maine
- Basic Life Support (BLS) certification

- Able to research and identify creative and effective solutions to patient, resources, or coordination issues
- Maintains complete and accurate documentation
- Proficient in computer knowledge, electronic medical records, typing skills, and ability to use phone system

- Able to flex hours to meet clinic/staff demands
- Maintains patient confidentiality
