Clinical Admission Liaison North Country Associates is in search of a Clinical Admission Liaison to manage referrals for multiple skilled nursing and rehabilitation facilities. This position will be responsible for assessing referrals from assigned hospital accounts to determine appropriateness for admissions as well as facilitate all aspects of a seamless transfer/admission process from acute to the sub-acute facility. The ideal candidate will have an outgoing personality, a strong clinical background and assessment skills with an attention to detail and customer service. Demonstrated working knowledge of admission process in nursing facilities is required.
