Kennebec Behavioral Health, one of the Best Places to Work in Maine in 2021, 2022 and 2023, is currently seeking a Registered Nurse with mental health experience to join our Behavioral Health Homes Case Management Team in Augusta.

Do you enjoy working as part of a team? Our Nurse Care Manager apart of the Behavioral Health Homes team will support clients of all ages in management of chronic medical conditions, completing preventative care needs and outreach to clients by phone calls and direct client appointments, as needed. The Nurse Care Manager will work as a part of a team with case managers and peer support specialists to provide a team-based approach to caring for our client population. This role includes other tasks such as care coordination amongst other healthcare professionals, transitions of care planning when hospital admissions/discharges occur and management of population health data to meet program regulations. The case managers will regularly consult the Nurse Care Manager to support client medical needs as they arise. Requirements:
· Maine licensure and mental health experience – RNC preferred but not required
· Experience in treatment of psychiatric disorders
· Knowledgeable in psychiatric pharmacology
