Purpose of Position or Position Summary:
Manages caseload of patients with mental Health diagnoses as part of the BHH team; Assist patients in attaining access to comprehensive primary and preventative health care services and linking to community resources. Provides supervision/support for Peer Supports/Case Managers, responsible for assisting with facilitation and follow-up needs of peer supervision groups and CQI meetings, media audits, referral wait list, assigning cases, and scheduling intakes in accordance with Mainecare policies and procedures, providing coverage of caseloads of supervises, as needed.
Position Minimum Requirements
Knowledge, Experience, and other Job Requirements
Education/License
4 year / Bachelor’s Degree in Social Work or related field, MHRT/C Certification required
Experience
2-5 yrs. Case Management experience
Able to serve both adults and children
Experience providing supervision preferred
