Full Time Medical Case Manager to provide medical case management services to people living with HIV/AIDS in the 5 northern counties of Maine, Washington, Hancock, Aroostook, Penobscot and Piscataquis. Case Manager will provide medical case management, adherence counseling, and support services at client’s homes and community locations and via telehealth.
