We’re currently seeking an experienced Registered Nurse (RN) to join our HomeCare North Team as a Case Manager. This position is 32 or 40 hours per week (candidate choice). Requires call, weekend, and holiday rotation with team members. This position is based out of Waterville, but staff must be flexible to work in/travel to areas surrounding the North Team’s territory (Skowhegan & Waterville).

Position Highlights:

• $15,000 Sign-On Bonus potential for nurses with at least 1 year of RN experience (New MGH hires only. Rehires within 1 year are not eligible. Bonus is paid out over 3 years as long as the employee stays in the job for at least 30 hours per week.)
• Case Managers have flexibility and autonomy when scheduling patient visits.

• We are deeply committed to providing ongoing education and support clinical certification and professional growth.

The ideal candidate will have:

• Active Maine RN license or ability to obtain the same in a timely fashion, required.
• At least 1 year acute or med/surg (or equivalent) nursing experience, required.
• Previous ED, ICU, CCU, or home care experience preferred.
• Valid driver’s license & acceptable driving record, required
• Strong technology/computer skills and previous experience with electronic medical records (EMR) preferred.


Job Description:

Consults with nursing staff and multidisciplinary team regularly to evaluate patient's status and appropriateness of care, including admission, length of stay, transfer and discharge.

Provides direct treatment and management of health conditions, including making recommendations to providers regarding additional services needed and referrals for community resources.

Plans, implements, and evaluates patient care plans based on patient assessment to optimize outcomes and maximize available resources. Monitors, records, and communicates patient condition as appropriate.

Participates in interdisciplinary team planning to collaborate with other disciplines to determine appropriateness of care and other services needed. Provides discharge education to patients.
