The Community Case Management team at Woodfords Family Services is growing and is currently seeking a qualified Adult Case Manager to join their program! This is a full-time, salaried position that's based out of our Westbrook office. Travel in and around Cumberland County as necessary is required.

Today, the organization provides an array of clinical, behavioral health and educational programs to more than 2,000 children, youth and adults living with autism, developmental disabilities, intellectual disabilities and/or mental health diagnoses across the state of Maine.

Position Summary

Case Management services are an essential resource for individuals living with intellectual and or developmental disabilities. They help in identifying consumer needs, connecting and coordinating appropriate services, and supporting navigation through various components of the individual person-centered-plans (PCP).

As a Case Manager, you will serve as a key communicator and advocate for adults and their families by ensuring all areas of development are being met and maintained. This will be achieved by meeting with adult consumers and the treatment team regularly to identify interests and strengths, vision for their desired lives, and goal setting geared toward attaining the highest level of independence.

Responsibilities:

  • Meets program productivity expectations as outlined by Program Supervisor.
  • Meets regularly with consumer in a variety of settings to assess consumer functioning, progress, quality of life, health and safety issues, and to address concerns in this area.
  • Seeks collateral contact with family and community providers to assess consumer needs and progress.
  • Identifies and reports any suspected or observed abuse and/or neglect of an adult.
  • Assists adults with maintaining a stable living arrangement and supporting their abilities to function in the community, vocationally, socially, financially and emotionally.
  • Provides triage to consumers with emergency situations and provides information to consumers on emergency preparedness.

Schedule: 8:00am - 4:00pm M-F with occasional evening hours. Opportunities to work remotely is available!


Qualifications

  1. Must possess a Bachelors Degree from an accredited four-year institution of higher learning with a specialization in psychology, behavior health, social work, special education, counseling, rehabilitation, nursing, or a closely related field.
  2. Must possess at least one-year's experience in the field of social services.
  3. Must possess a valid State of Maine driver's license and have use of a personal vehicle and proof of auto insurance.
  4. Must possess good organization, communication, decision-making, and human relation skills.
