We have a new opening for an RN Care Manager. The RN Care Manager works with the practice team to provide proactive, comprehensive care management within the patient-centered medical home model. Together with clinical and administrative leadership and other members of the Practice Team, will be responsible for attaining and maintaining Patient Centered Medical Home NCQA Recognition for the practice.
Knowledge, Skills & Abilities:
Ability to provide individualized, patient-centered care planning
Ability to identify social determinants of health (SDOH) impacting the health and care planning for each patient
Knowledgeable of the types of care that can be safely provided in a community setting and providing this information to patients and families
Understanding of healthcare settings and resources in the community such as outpatient clinics, skilled nursing, or acute rehabilitation hospitals, homecare
Understanding and educating patients and their families of their right to choose their post-acute services and documentation of providing choice
Monitoring and updating of the patient’s condition for necessary changes in the post-acute transition plan
Ability to use electronic communication software(s)
Leadership and negotiation skills are essential in coordinating care within the hospital and the community
Duties and Responsibilities:
Ability to conduct an assessment of patient needs, identify resources required to transition patients to the next setting, and secure the necessary resources
Collaborate, educate, and lead an interdisciplinary team of professionals in daily care rounds/care conferences/care planning
Excellent communication skills in written, verbal, and electronic systems such as email or with software applications currently used with the hospital
Ability to work with community organizations to secure durable medical equipment, health insurance authorizations, arrange transportation
Participate in the length of stay of an inpatient hospital population
Knowledge of hospital reimbursement methods and quality measures applicable to the care management department including: hospital readmissions, length of stay, patient returns to the emergency department, medical necessity denials, observation and inpatient levels of care
In order for us to continue to provide exceptional patient/customer experiences, we need you to have the following:
Associate or Bachelor degree-prepared Registered Nurse with 3-5 years clinical experience in medical/surgical nursing, home care, hospital case management and/or previous utilization review experience
Certification by a nationally recognized case management or utilization review organization preferred
Licensure: Current Licensure as a Registered Nurse in the State of Maine
