Hours: 7:00am-5:00pm (4 10-hr. shifts)
This position will support Northern Light Eastern Maine Medical Center at its Internal Medicine clinic in Bangor and Family Medicine clinics in Bangor and Orono.
POSITION SUMMARY: The Integrated LCSW position is an exciting opportunity to use evidence-based knowledge and acquired skills as a key member of a dynamic, collaborative team in an integrated primary care setting. This person is a clinical leader, consultant and resource at the medical practice in program development, in guiding group decision-making, in implementing plans, and improving quality of care. He or she is an enthusiastic learner who actively seeks research and knowledge to ensure current, evidence based practice while adhering to key components of our integrated behavioral health model.
The Integrated LCSW will be skilled in establishing rapport and maintaining effective working relationships with diverse individuals, including patients, family members, and other providers. The integrated LCSW serves as an effective member of an inter-professional team, helping other providers on the team to quickly conceptualize a patient's strengths, healthcare problems, and an appropriate plan of care. They adapt interventions to the work flow and pace that typically characterizes the provision of primary care, including rapid assessment, brief treatment, and a high daily volume of patient contacts. They advocate for, teach, and support whole health self-management and recovery approaches.
They will demonstrate a fundamental belief in the value and effectiveness of brief interventions to improve health through practice patterns and communications with patients, family members, and other members of the health care team. They collaborate with the healthcare organization, behavioral health leadership, and other local healthcare agencies to continuously assess and improve service system design. EDUCATION / LICENSUREREQUIREMENTS: Active Maine LCSW license required.
EXPERIENCE REQUIREMENTS:At least two (2) years' experience in a clinical practice required;supervisory experience preferred. Experience in hospital-based clinical treatment servicespreferred.
REGULATORY REQUIREMENTS:Maintains patient confidentiality at all times. Represents the organization in a positive and professional manner in the community. Maintains regulatory requirements, including Joint Commission, Federal, and State. Complies with all organizational policies regarding the EMHS Code of Conduct and ethical business practices. Completes Healthstream Mandatory Education on a yearly basis. Attends all mandatory safety-related education programs. Follows safety procedures to prevent injury to self and others. Carries assigned keys and wears hospital ID badge at all times. Refrains from the abuse of chemicals.
OTHER SKILLS REQUIRED:Must possess good oral and written communication skills in the English language. Ability to be flexible and exercise independent judgment. Demonstrates collegial and multi-disciplinary relationships along the treatment continuum.
PHYSICAL REQUIREMENTS:Visual acuity, color vision, hearing acuity, and tactile sensation are necessary to respond to patient response and requests. The ability to read and speak English, to communicate and document information. Must possess the manual dexterity and a degree of body strength and flexibility to respond to aggressive patient behavior. The ability to reach and stretch intermittently. Time may be spent sitting at a computer, collaborating with other healthcare team members. Standing and walking comprises some of the workday.
