Our Hancock County location has a great position for a nurse practitioner wanting to make a positive difference in someone's life! You would be responsible for performing face to face encounters with Hospice clients, assisting the Hospice Interdisciplinary Team in determining ongoing hospice eligibility. You would focus on client safety, ensuring timely and correct disposition of all patients, including appropriate coordination of care and client self-management opportunities. Utilizing the latest information technologies and telecommunication systems, VNA provides you with a variety of nursing experiences, ranging from caring for clients with medical/surgical needs as well as caring for clients with hospice and palliative care needs. In addition you would play an integral role in collaborating with other members of the client care team. Bachelor's degree from an accredited school of nursing Certification by the national certifying body appropriate to his/her clinical specialty Three years acute care nursing experience Current ME license to practice as a registered professional nurse Approval from the Board of Nursing to practice in the advanced role Current BLS for Healthcare Providers Computer competency and evidence of effective communication skills, using verbal, written and telephonic means; ability to make decisions regarding priority of work, strong interpersonal skills, and ability to understand specific instructions Valid driver's license with reliable transportation and VNA required auto liability insurance.
